A woman who was thought to be sleeping by hospital staff in London has died.

The death of 22-year-old Shannara Donnelly at a hospital in North London revealed significant neglect within the healthcare system. Despite her condition requiring constant supervision, staff monitored the young woman for over an hour only through cameras and windows. Although Donnelly needed '24/7' care, adequate observation was not provided. The NHS Foundation acknowledged shortcomings in risk assessment and care errors.

The case heard at the North London Coroner's Court highlighted serious negligence in the healthcare system. The tragic death of 22-year-old Shannara Donnelly, who should have been under constant supervision, occurred because hospital staff failed to intervene for over an hour.

In the incident that took place at Chase Farm Hospital, Donnelly collapsed on the floor of her room. Healthcare personnel monitoring the situation via hospital cameras did not intervene for more than an hour, mistakenly believing the young woman was asleep. The investigation indicated that Donnelly "took her own life."

The young woman, known as Jade by her family, was reported to be a victim of sexual abuse, had a history of alcohol addiction, and occasionally experienced auditory hallucinations. Donnelly, who faced difficulties during the Covid quarantine period, had lost three close family members before her death. She was an active member of the Army and Police student unions and the Royal British Legion.

Despite being noted in health records that Donnelly should not have been admitted to Chase Farm Hospital, she was accepted after being arrested due to suicidal tendencies prior to her hospitalization. Although her condition required "one-on-one" care and "24/7" supervision, the staff only conducted insufficient observations through cameras and monitoring windows.

The Barnet, Enfield, and Haringey Mental Health NHS Foundation acknowledged the "shortcomings" in care and stated that the risk assessment was inadequate. Mark Pritchard, the Foundation's director, noted that the use of CCTV was an insufficient method of observation and apologized to the family.

During the questioning by the family's lawyer, Ross Beaton, it was revealed that the standard of patient care was not met and that the conditions for Level Three supervision were not fulfilled. The investigation is still ongoing.

Donnelly's mother, Kerry, stated, "Jade always loved to do different things and put her whole heart and soul into whatever she did. A beautiful leaf has fallen from our family tree, but Jade's memories will live on in our hearts forever," expressing that they would keep her daughter's memory alive.
