A 60-year-old man in Üsküdar had his life turned into a nightmare after he was infected when he bit the hand of one of the thieves who entered his shop. The man's arm, which became infected, was saved from amputation thanks to approximately 90 hours of hyperbaric oxygen therapy. This rare case caused by a human bite has surprised even doctors and is preparing to enter the medical literature.

A thief broke into the shop of N. U. (60) in Üsküdar 1.5 years ago. One of the two thieves managed to escape, while the other, who was caught, bit N. U.'s left hand and fled. INFECTION CAPTURED N. U. stayed here for 15 days receiving intensive infection treatment. During this time, the wound on his hand progressed, and his arm swelled to twice its normal size up to the elbow. The hospital's Department of Underwater and Hyperbaric Medicine intervened for his arm, which was at the stage of being amputated, and N. U. was urgently taken for hyperbaric oxygen therapy.
JUST ESCAPED FROM LOSING HIS ARM Thanks to the efforts of the team led by Dr. Yavuz Aslan, which lasted about 3.5 months with subsequent physical therapy, N. U. was saved from losing his arm.
DOCTORS WERE EVEN SURPRISED Specialist Doctor Yavuz Aslan noted that the incident surprised them as well: "We noticed that the patient had a large ischemic, necrotic wound on his left hand, meaning that its blood supply was disrupted. Within a few days, there was severe swelling in his forearm that spread up to the elbow," he said.
WENT THROUGH PRESSURE THERAPY FOR DAYS Aslan stated, "We particularly emphasize hyperbaric oxygen therapy for such wounds, as it helps with infection treatment. We planned hyperbaric oxygen therapy for the patient, lasting 2 hours with 1 session per day. After about a week, we were able to debride the wound, meaning we could clean it. We performed the wound cleaning, removed the necrotic tissues, and special collagen materials were placed where the tendons were exposed. Over a period of about 1.5-2 months, the patient's wound completely healed. He was able to use the functions of his hand. We saved the patient's hand and forearm from amputation. Later, we referred him to the physical therapy department to fully restore his hand movements," he explained.
READY TO ENTER THE LITERATURE Specialist Doctor Aslan shared that he had never treated a case of human bite throughout his career and that the case surprised them as well. He stated that the bacteria found in the human oral flora normally do not cause any problems, but when they enter the subcutaneous tissue, they can lead to severe conditions. Aslan concluded his remarks as follows:
"There are bacteria in the human oral flora, such as Eikenella or Kingella. In the samples we took, these types of bacteria had also multiplied. They cause severe swelling and block lymph flow, leading to strong edema. This disrupts circulation and delays wound healing. The volume of the hand had increased by about 1.5 - 2 times, especially in the area from the wrist to the elbow. It is very difficult for antibiotics to clean the abscess. A surgical cleaning procedure needs to be performed there. Hyperbaric oxygen therapy has a very strong edema-reducing effect. Surgical cleaning of the wound needs to be done simultaneously with hyperbaric therapy. We saved the patient from amputation by healing over a period of 1-1.5 months. We had never treated a case of human bite before. This was our first. We saw how severe a wound a human bite can cause. We saved the patient's hand from amputation in a way that we had never seen in the literature before. There are wounds related to human bites in the literature, but they are smaller wounds; there are wounds that lead to amputation but could not be saved. Because we added hyperbaric oxygen therapy at the appropriate stage, the patient's hand was saved from amputation. He underwent nearly 35-40 sessions of hyperbaric therapy, and after the wound closed, we continued for 5 more sessions. We are in the process of publishing this as an article. Especially in cases where teeth come into contact with the skin and create deep wounds, pathogens that normally do not cause any problems in the teeth can enter under human skin and cause infections with serious consequences."
