Some details in the report are chilling. It was documented that the lifeless body of a baby girl, after an autopsy, was "accidentally" thrown into medical waste by laboratory workers. Go home, take a painkiller, have a warm bath, and pull yourself together," refusing medical help. A toxic environment of bullying by senior staff towards other employees was also found within the hospital.

Following the scandal, the UK Department of Health announced an emergency action plan.
