"GAS LEVEL REACHED MAXIMUM"

Initial inspections on the ship revealed that the gas level in the slop tank had reached maximum levels, and it was determined that the incident occurred because the deceased and injured crew members entered the tank without precautions. The second captain of the ship, identified in the expert report as responsible, has been detained and arrested by the Duty Magistrate.
